-----------------------------------------

PLEASE NOTE YOU MUST BE A MEMBER OF NFO before applying for insurance.

Be aware you will need to answer the following questions during the application process:

1. Is the insurance certificate for a particular event? If yes, you will need to provide all relevant information about the event including your role in the event, the date, time, location, frequency, and whether or not food or alcoholic beverages will be served at the event. A weekly dance group can be considered an event and should be indicated as such.

2. If the answer to #1 above is "no" - are you simply requesting a certificate which shows you have insurance coverage?

3. Has anyone asked that you show proof of coverage? If yes, you will need to provide the complete address information of that party.

4. Has a third party (usually a venue such as a church or community center where the dance event is held) requested listing on the certificate as an additional insured? If so, you will need to provide all address information about this party as well as its role, if any, in the event.

5. You will also be asked to provide any information that is unique to you and/or your dance group. An example might be that you are a freelance dance teacher who teaches in many different schools.
